Chasing feedback from all the different samples you request can be a lot to juggle. So, we’ve launched a new Dysphagia Sample Feedback Service to make things easier for you.
Here’s the idea: whenever you request a sample from the www.slodrinks.com/clinicians-library, we will include an information leaflet with the same unique four-digit code you were sent.
The leaflet below is an example of one sent to a Nursing Home or Care Facility. A similar leaflet is included with samples sent direct to your patients at home.
The person trying the sample just has to go to www.slodrinks.com/feedback, to follow two quick steps to share their thoughts.
Right after it’s been completed, you’ll receive a confirmation email* with what they liked, so you have what you need to make prescription decisions confidently.
We’ve also included a link to the Prescription request form. That contains PIP codes and recommended number Slõ Syrup bottles to prescribe for the daily fluid intake you recommend.

If the sampler or their carer would rather call us with their feedback, that works too.
They simply call us on 07583 803 175 and leave a message and we’ll make sure you get all the details.
As sample orders are sent will within 24 hours of receiving your request, they will arrive quickly and you should receive an email from us – if the recipient has used our service.
If you don’t hear from us 7 days after submitting your request, please contact them directly.
